Yes, but we are gradually dropping insurance and moving toward self-pay in order to serve you better without the restrictions imposed by insurance and case reviewers. Please call us with your insurance information to verify coverage.
For out-of-network claims, we can provide you a Superbill to submit for reimbursement yourself. Amount of reimbursement to you varies and depends on your insurance. Payment is not guaranteed. This is one of the reasons we are moving toward self-pay.

Absolutely! Our purpose is to help you get back to the life you want. At Starr MFR, we accept self-pay or out of network insurance (if your plan allows).

Self-pay allows you to receive the care that you need and deserve without the restrictions placed upon your treatment by insurance.
You and your therapist have control of your healing experience without 3rd parties dictating your treatment, such as limiting the number of sessions and disrupting treatment while waiting for authorizations.
Sessions are not wasted on reexaminations to justify to a third party (insurance) that you need treatment. You know you are in pain, have an injury, or need care. It is not fair that someone who never sees you tells you what you need or get.

No; Ohio allows for direct access to physical therapy without a physician prescription. We will send your Plan of Care to your physician after your initial examination.
Patients with Medicare should bring a physician prescription to the initial exam.

Please bring valid identification, insurance card (for physician referral visits only) and completed new patient forms. If you wish to fill out the patient forms in office, please arrive 15 minutes early to your appointment.
Please bring or wear comfortable, loose clothing.
